Please arrive 15 minutes prior to your appointment time to complete our patient forms.
This paperwork provides us with general information about yourself and your health goals.

New Patient Health History

Consultation

Once the paperwork has been completed, you will have a consultation with Dr. Naas to discuss your health-related concerns and goals.

History and Examination

In order to properly evaluate your health, Dr. Naas will take a detailed health history and will perform an examination to determine how you can be helped with chiropractic care. If chiropractic care is not the answer for you, Dr. Naas will refer you to the appropriate health care professional.

Same Day Adjustment

Once finished with the examination and all appropriate studies, most patients will receive an adjustment during their first visit.

Home Instructions

Prior to leaving our office, you will be given instructions on how best to manage your condition at home. This may include the use of ice or heat, avoidance of certain activities or positions and home exercises.

Office Accessibility

There is an accessible entrance in the back of the building on the west side. This leads to a ramp which you can use to access the elevators in the foyer. Call before your appointment and we will be glad to provide you with any additional assistance necessary.
